- List all individuals in your household, beginning with yourself and your spouse if applicable. Include children who you provide more than half of their support from July 1, 2010, to June 30, 2011.
- Include other people if they currently live with you, you provide more than half of their support, and you will continue to do so during the specified timeframe.
- For each person listed, provide their first and last name, age, relationship to you, and whether they live at home and receive more than 50% support.
- Review all entered information for accuracy, then sign and date the form before submission.
