8. In the applicant information section, enter your company name, address, contact information, and details about your insurance history and vehicles.
9. Complete the coverages requested section by marking the appropriate limits for auto liability, physical damage, and any additional coverages.
10. Fill out the equipment and driver information sections as applicable. Make sure to attach additional sheets if you have more than the space provided for equipment or drivers.
11. Finally, review all entries for accuracy, sign the form acknowledging the fee disclosure, and indicate whether you will provide MVRs or if you would like NASTC to obtain them on your behalf.
12. Once all the sections are completed, save your changes, download a copy for your records, and send the completed application via email or fax to the provided contacts.
