- If an advisement PIN is required, be sure to enter it in the designated field to avoid processing delays.
- Finally, ensure you sign and date the form in the provided areas. Additionally, advise your advisor and dean to review and sign the document before submission.
- Once you have filled out all required fields and obtained necessary signatures, submit the completed form to the Office Of Registration & Records either in person or by fax.
