7. Enter the company phone number, extension (if applicable), and fax number. This ensures the association has all means to reach your company.
8. Provide the company email address. This should be a reliable email contact for your organization.
9. In the section for products or business of the company, describe what your company does concisely. This helps the association understand your business.
10. Indicate your job title, position, or occupation at the company on the next line. This identifies your role within the organization.
11. In the job description field, provide a brief overview of your responsibilities. This detail will give context to your application.
12. Fill in how long you have been in your current position and choose your preferred mailing address by circling either home or company.
13. Indicate how long you have been in the trade and the number of employees you supervise, along with the count of trucks, tractors, and trailers associated with your work.
14. For references, secure the signatures of two current members familiar with your work, along with their printed names.
15. Describe your expectations from the association in the designated section. This information helps tailor the membership experience.
16. Add your signature and the date at the bottom of the form to validate your application.
17. Lastly, ensure you attach the payment as stated in the application. You can include a cheque or complete an e-transfer as instructed.
18. Once all fields are completed, save your changes, download the form, print it for your records, or share it as necessary.
