5. Enter your business address, which must be the physical location of your operations. Avoid using a P.O. Box.
6. Move to Part 2, 'Insurance'. Mark 'YES' or 'NO' for statements regarding vehicle weight and the types of commodities transported. Complete the relevant section based on your responses.
7. Continue to Part 3, 'Interstate Operations'. Indicate whether you hold a USDOT Number and provide it if applicable. Confirm if you have a Unified Carrier Registration for the current year.
8. Complete Part 4, 'Verification', by signing and dating the application. Ensure that the individual signing has the authority to file on behalf of the applicant.
9. Review all sections of the form to ensure it is fully completed and accurate. Submit the application along with the required non-refundable application fee via US Mail, as faxed or emailed applications will not be processed.
10. Once your application is submitted, keep a copy for your records. You can also track the application status as necessary.
