7. Select the number of unofficial transcripts you are requesting, keeping in mind that you are limited to five.
8. If you wish to update your address on file with the address you just provided, select the corresponding checkbox.
9. If you are a former student, specify where you would like your unofficial transcript to be sent by filling in the recipient's name or institution, street address, city, state, zip code, contact email, and phone number.
10. Acknowledge your authorization for UCI to provide your unofficial academic transcripts by signing in the designated section and entering the date of your signature.
11. Once your form is completed, you may submit your request by mail or email. For mail, send it to the provided address; for email, attach a scanned copy of the form and send it to the specified email address.
