(1) "Abandoned" means the parents of a child have failed to visit or maintain contact with the child for more than ninety days, regardless of whether the parents resume contact with the child after that ninety-day period. (2) "Child" means an individual who has not attained eighteen years of age.

You must visit a Driver Service Center to make changes to your name. A form for an adult name change is available in the Office of the Clerk and Master, or may be downloaded from chanceryclerkandmaster.nashville.gov. Chancery Court requires the petition to first be filed. Once filed, the case is randomly assigned to one of the Chancellors.
