Current American Thyroid Association guidelines recommend treating subclinical hypothyroidism only if the TSH is above 10, regardless of age. This review adds more evidence to favor treatment of subclinical hypothyroidism in younger patients but restrict treating older patients unless their TSH is >10.

The ATA guidelines classify the US appearance of thyroid nodules into benign or low, intermediate or high suspicion of malignancy. The BTA guidelines classify the US appearance of thyroid nodules as U1 (normal), U2 (benign), U3 (indeterminate), U4 (suspicious) or U5 (malignant).

Adults should be screened for thyroid dysfunction by measurement of the serum thyrotropin concentration, beginning at age 35 years and every 5 years thereafter. There is insufficient evidence to recommend periodic, routine screening for thyroid dysfunction among asymptomatic persons using serum TSH levels.

Hypothyroidism refers to any condition in which the thyroid gland produces too little thyroid hormone. Symptoms may include feeling run down, slow, depressed, sluggish, cold, or tired and having dry skin and hair, constipation, muscle cramps, or weight gain. Women may have an increased menstrual flow.

Eating foods that have large amounts of iodine—such as kelp, dulse, or other kinds of seaweed—may cause or worsen hypothyroidism. Taking iodine supplements can have the same effect. If you are pregnant, you need more iodine because the baby gets iodine from your diet.
