9. Complete section F only if your committee engages in campaign activity related to ballot issues. Indicate the support or opposition for each ballot issue.
10. In section G, list any individuals other than the treasurer who are authorized to sign checks, providing their names, residential addresses, and phone numbers.
11. Once all sections are completed, ensure you include the original signature of the treasurer in ink and date the form.
12. You can then save your changes, download the completed form, print it for submission, or share it as required.
