- If applicable, fill in section E with the election year, office or district, candidate's full name, and specify whether you support or oppose each candidate.
- Complete section F only if your committee engages in campaign activity related to ballot issues. Indicate the support or opposition for each ballot issue.
- In section G, list any individuals other than the treasurer who are authorized to sign checks, providing their names, residential addresses, and phone numbers.
- Once all sections are completed, ensure you include the original signature of the treasurer in ink and date the form.
