- In Part 3, report total household gross income. List each participant’s name in Column A, and next to each name in Column B, indicate the gross income from various sources (work, welfare, Social Security, etc.) and the frequency of receipt (weekly, bi-weekly, monthly).
- In Column C, check the box if any individual listed has no income. Ensure all applicable income sources are documented, and if necessary, use additional sheets for listing names.
- Part 4 requires an adult member to sign the form. If Part 3 is completed, the last four digits of the Social Security Number must be provided, or indicate if there is no Social Security Number.
- In Part 5, optionally mark the participant's ethnic and racial identities. Note that this section is for official use and does not affect eligibility.
- After completing all sections, review the form for accuracy, then save changes, download, or print your filled form as needed.
