13. If you have immediate relatives employed at Purdue, indicate 'Yes' and list their names and departments if applicable.
14. In the exemptions section, apply any relevant codes if applicable.
15. Fill out the itemized payment section, listing all applicable fees, expenses, and quantities related to your payment.
16. In the Certification of Payee section, read and agree to the terms by providing your signature.
17. Fill in your printed name and today's date.
18. Complete the account information section by providing the G/L account, order number, WBS element, and any earmarked funds as required.
19. Ensure the verification of receipt of deliverables and/or services is signed by the authorized individual.
20. Review the completed form for accuracy and completeness before submission.
