10. Finally, for users who are aware of potential fees, initial to confirm acceptance of the $25 surcharge for any occurrences of insufficient funds.
11. Sign the form with your Account Holder Signature, input your legal name again, and date the form.
12. After completing all sections, review your information for accuracy. Save changes, download a copy, or print the form as needed to retain a record.
