Get IRS 8889 2022-2024

If, during the tax year, you are the beneficiary of two or more HSAs or you are a beneficiary of an HSA and you have your own HSA, you must complete a separate Form 8889 for each HSA. Enter “statement” at the top of each Form 8889 and complete the form as instructed.

We'll automatically fill out Form 8889 if you and/or your employer contributed to an HSA (usually indicated by code W in box 12 of the W-2), if you used funds in your HSA (reported on Form 1099-SA), or if you inherited an HSA.

When filing your taxes, you are required to file IRS Form 8889 if you (or someone on your behalf, including your employer) made contributions to your HSA, or if you received HSA distributions for the year. For other conditions, see the IRS instructions for the 8889 Form.

You will need to upgrade to the Deluxe version because of the employer HSA contribution. The IRS requires you to include Form 8889 with your Form 1040 to show your eligibility for the contribution.

You must always file a Form 8889 in any year you or an employer contributes money to your HSA or you make withdrawals from the account. The deduction you calculate on Form 8889 is taken on the first page of your income tax return.
