- Provide all course details including school name, semester/year, course name, number, credits, start and end dates, and whether it is an online course (yes/no).
- Sign at the bottom of the form to acknowledge your agreement with the SAS transfer policies and include the date.
- Upon completion, submit the form as instructed and wait for the official review notes, which will be provided to your Rutgers email within two weeks.
