Get 2023 Form 1095-A

By Jan. 31 of each year, Covered California sends the federal IRS form 1095-A Health Insurance Marketplace statement to members. This form is used to: Provide information for your federal taxes.

If you have not received your Form 1095-A or you received an incorrect 1095-A, you should contact the Marketplace from which you received coverage. If you receive a corrected or voided Form 1095-A, you may need to amend your return.

If you enrolled in coverage through the Marketplace you will need the information on Form 1095-A to complete Form 8962 to reconcile any advance payments of the premium tax credit or claim the premium tax credit, and to file a complete and accurate tax return.

If you didn't get your Form 1095-A in the mail, or you can't find it, check your HealthCare.gov account: Log into your HealthCare.gov account. Under "Your Existing Applications," select your 2022 application. Select “Tax Forms” from the menu on the left.

Your proof of health coverage form (1095-B) will arrive soon Health Net will mail Form 1095-B to our members starting January 31, 2023. This form is not required when you file your federal or state 2022 tax return. But, we do suggest that you keep this form with your tax information.

How to find your 1095-A online Log in to your HealthCare.gov account. Under "Your Existing Applications," select your 2022 application — not your 2023 application. Select “Tax Forms” from the menu on the left. Download all 1095-As shown on the screen.

If you purchased coverage through the federally facilitated Marketplace and you set-up a HealthCare.gov account, you can get a copy of Form 1095-A, Health Insurance Marketplace Statement online from your account.
