Also, fill out the IRS Identity Theft Affidavit, Form 14039.

Mail or fax your form and the attached copy of your ID to the Internal Revenue Services, PO Box 9039, Andover, MA, 01810-9039 or fax it to 855-807-5720.

The IRS wants you to submit Form 14039 when you believe someone has unauthorized access to your personal information.

Complete Form 14039, Identity Theft AffidavitPDF, attach it to the back of your completed paper tax return and mail to the IRS location based upon the state you reside. If you prefer, you have the option to submit the Form 14039 online and mail your paper return separately.

Filing Form 14039, Identity Theft Affidavit Taxpayers can also complete the form online at the Federal Trade Commission and the FTC will electronically transfer the Form 14039 – but not the tax return – to the IRS.
