Medicare takes approximately 30 days to process each claim. Medicare pays Part A claims (inpatient hospital care, inpatient skilled nursing facility care, skilled home health care and hospice care) directly to the facility or agency that provides the care.

After a health care provider treats a Medicare patient, the provider sends a bill to Medicare that itemizes the services received by the beneficiary. Medicare then sends payment to the provider equal to the Medicare-approved amount for each of those services.

The Medicare Administrative Contractors, (MACs), intermediaries, and carriers are responsible for processing claims submitted for primary or secondary payment and resolving situations where a provider receives a mistaken payment of Medicare benefits.

Providers can enter data via the Interactive Voice Response (IVR) telephone systems operated by the MACs. Providers can submit claim status inquiries via the Medicare Administrative Contractors' provider Internet-based portals. Some providers can enter claim status queries via direct data entry screens.

Fill out a "Redetermination Request Form [PDF, 100 KB]" and send it to the company that handles claims for Medicare. Their address is listed in the "Appeals Information" section of the MSN. Or, send a written request to company that handles claims for Medicare to the address on the MSN.

The EDI 276 transaction set is a Health Care Claim Status Inquiry. It is used by healthcare providers to verify the status of a claim submitted previously to a payer, such as an insurance company, HMO, government agency like Medicare or Medicaid, etc.

Section 50 of the Medicare Claims Processing Manual establishes the standards for use by providers and suppliers (including laboratories) in implementing the Advance Beneficiary Notice of Noncoverage (ABN), Form CMS-R-131.

Your provider sends your claim to Medicare and your insurer. Medicare is primary payer and sends payment directly to the provider. The insurer is secondary payer and pays what they owe directly to the provider. Then the insurer sends you an Explanation of Benefits (EOB) saying what you owe, if anything.

Overview. Your Medicare Part A and B claims are submitted directly to Medicare by your providers (doctors, hospitals, labs, suppliers, etc.). Medicare takes approximately 30 days to process each claim.

You can check your Medicare status in several ways: Log into your MyMedicare.gov account. Call the Social Security Administration at 1-800-772-1213. Visit your local Social Security office. Contact your State Health Insurance Assistance Program (SHIP). Log in to your My Social Security account.
