- Multiply the result from line 4 by 0.005 to determine your deduction for new tires sold in excess of 3,000.
- Add lines 5 and 6 to get your total deduction.
- Finally, subtract line 7 from line 1 to calculate the final amount owed on total.
- In Section III, certify the report by providing your authorized signature, date of signature, printed name, and title.
- Section IV involves submitting your payment. Sign and date the report and, if this is your final report, list the closure date. Keep a copy for your records and send your payment as directed.
- If applicable, even if you paid the $1.00 fee to your tire distributor during the reporting period, you still need to submit the form with '0' on line 8 and provide any purchase invoice copies.
