17. Indicate the quantity of each item along with the applicable unit of measure.
18. Fill in the HTSUS number or Schedule B number for each exported or destroyed item.
19. Select the type of drawback being filed.
20. Provide the signature, title, and date from the authorized person who is binding the company to the document veracity.
21. Review all entries for accuracy before saving changes. You can then download, print, or share the form as needed.
