Log into your CAFÉ portal account at .dcfs.la.gov/cafe. Click on “My Applications” tab. Click on “Upload Documents.”.

Eligible households can receive food assistance through regular SNAP or through the Louisiana Combined Application Project (LaCAP). For more information on SNAP and other services available through the Department of Children and Family Services (DCFS), call 1-888-LAHELP-U (1-888-524-3578).

If you lose or damage your EBT card, or if you need to cancel your EBT card, call EBT Customer Service at 1-888-997-1117, 24 hours a day, 7 days a week. Customer Service Number at 1-888-997-1117.

Choose one of the following options: Mail the completed form to the Document Processing Center: DCFS Economic Stability. P.O. Box 260031. Baton Rouge, LA 70826. Fax the completed form to: (225) 663-3164. Drop off at the nearest local office.

If you already have an account, enter your User ID and password and click the LOGIN button to Sign-In which will take you to the MyAccounts page. On the MyAccounts page, click My Simplified Reporting which will allow you to submit a Simplified Report.
