How to fill out the People With Disabilities online

- Proceed to Section 4 and ensure that a licensed physician, APRN, or PA completes the medical statement. They must indicate the disability, specify whether it is temporary or permanent, and mention the length of the disability.
- Finally, review all provided information for accuracy. You can then save the completed form, download it for your records, print a hard copy, or share it with the necessary parties. If mailing, include a check or money order for any applicable fees.
