An Entry of Appearance is a legal document that says that an attorney represents one party in a case. It is a representation to the court that an attorney represents one party or the other.

An Appearance form (JD-CL-12) is the official court form filed with the Court Clerk that places your name and address on file with the Court. It also tells the court that you know about the case and that you are going to represent yourself in the case.

An entry of appearance shall state (1) the case style and number; (2) the identity of the party for whom the appearance is made; and (3) the name and current office address and telephone number of the attorney.

The defendant may make his or her appearance by filing a motion within the 30-day period, in which instance an answer or another appropriate motion shall be filed within the time the court directs in the order disposing of the motion.

If a person ignores the ticket or fails to appear in court on any court date scheduled, the judge will enter an ex parte judgment of conviction and assess a fine against that person in the amount set by the Illinois Supreme Court.

o An Appearance is a form you file with the Circuit Clerk to tell the court and the other parties that you are participating in the court case.

o Someone sues you and you want to participate in the court case. If you do not file an Appearance and go to court as needed, the court case might go ahead without you and the court might rule against you.

Signing an entry of appearance and waiver of service only means that the party signing the document is giving up his/her right to be served by a sheriff's deputy. It does not mean they agree with the lawsuit or are giving up their right to notice of when any hearings are to be held.
