- If you are applying to receive PTO donations, fill out the Request to Receive Shared PTO Funds section. Indicate the qualifying reason for your request and confirm that you have exhausted your PTO.
- If applicable, complete the Physician’s Certification section and provide the necessary medical information and signatures required for your application.
- Once all required fields are completed, save your changes. You can then download, print, or share the completed form as necessary.
