Follow the steps to successfully complete the PTO Donation Program form

1. Press the ‘Get Form’ button to access the PTO Donation Program application. This will allow you to begin filling out the necessary fields in the form.
2. Provide your employee name and the date on the application form. This information is essential to identify you as the donating employee.
3. Indicate your current PTO balance. This will inform Human Resources of how many hours you have available for donation.
4. Specify the number of hours you wish to donate by entering that number in the designated field. You may also note how many hours you designate for a specific person.
5. If you prefer to remain anonymous, check the box indicating your wish for confidentiality regarding the donation.
6. Sign the form to confirm your understanding that the donation is irrevocable and will be used to assist another employee in need.
7. If you are applying to receive PTO donations, fill out the Request to Receive Shared PTO Funds section. Indicate the qualifying reason for your request and confirm that you have exhausted your PTO.
8. If applicable, complete the Physician’s Certification section and provide the necessary medical information and signatures required for your application.
9. Once all required fields are completed, save your changes. You can then download, print, or share the completed form as necessary.
