- If filing more than one year after the child’s birth, include two (2) originals and two (2) photocopies of a notarized Affidavit of Delayed Registration explaining the reasons for the delay.
- Once all documents are completed and assembled, mail the requirements to the Philippine Consulate General in Chicago. Include a self-addressed return envelope with the necessary postage.
- Lastly, ensure to include the non-refundable processing fee: $25.00 for the ROB only, or $50.00 for ROB and related affidavits. Personal checks and credit cards are not accepted.
