- Complete Section 3 only if you have indicated insurance mediation activity in Section 2. Provide details for the Insurance Mediation Officer, including surname, forename, title, SRA number, date of birth, and role start date.
- In Section 4, an Authorised Individual manager must complete the declaration. Provide surname, forename, title, SRA number, date of birth, and include a signature with the date. If the form is completed electronically, ensure to tick the box confirming the declaration.
- Once all sections are complete, save the changes to your form, then download, print, or share it as needed.
