7. Complete the section for the appliance or item description. Indicate the brand, code number on the box, quantity in the pack, the number of packs ordered, and calculate the total for this section.
8. Review the payment section. Include the total for postage, annual fees, any donations you wish to make, and ensure your total payment is accurate.
9. If paying by credit card, fill in the cardholder's name, card number, expiry date, and sign where indicated. Alternatively, you may choose to pay via direct deposit or cheque.
10. Once you have filled in all sections accurately, review the entire form for completeness, then save your changes, download, print, or share the form as necessary.
