Visual acuity is a psychophysical test that measures central visual function. Low contrast VA (LCVA), tested using grey letters on a white background, has increased sensitivity for neurological visual pathway disease over high contrast VA (HCVA), which is tested using black letters on a white background.

Visual acuity refers to the clarity of vision and the ability to distinguish details of objects. Contrast sensitivity is the ability to distinguish an object from its background.

Visual acuity determines the sharpness and clarity of your vision from a distance. Contrast sensitivity refers to your ability to differentiate between an object and its background. Contrast sensitivity is also more sensitive than visual acuity, which makes monitoring it potentially more important.

Low contrast visual acuity (LC-VA) is known to be able to capture visual loss not seen in conventional high-contrast visual acuity (HC-VA) in MS. Therefore, to increase the sensitivity of ON detection, we investigated the advantage of LC-VA over conventional HC-VA.

LEA SYMBOLS® Low Contrast Test, 10M Test for quick measurement of low contrast visual acuity at 25%, 10%, 5%, 2.5%, and 1.25% contrast by measuring the distance where the symbols are seen. 10M symbols correspond to visual acuity 0.10 at 1 m, 0.05 at 0.5 m and 0.3 at 3 m. VA values are thus easy to calculate.

Contrast is usually expressed as percent, then the ratio is multiplied by 100. The maximum contrast is thus 100% contrast. The symbols of the visual acuity charts are close to the maximum contrast. If the lowest contrast perceived is 5%, contrast sensitivity is 100/5=20.

At all blur and luminance levels, visual acuity decreases gradually for contrast levels down to 20%, and decreases sharply for lower contrast levels. Over the range of background luminances we tested (75.0 to 0.075 cd/m2), visual acuity decreases linearly with reductions in luminance.

The most common contrast sensitivity test is the Pelli-Robson contrast sensitivity chart. This test is a chart with different capital letters organized in horizontal lines. The contrast decreases with each line. The person will move down the chart to determine the least level of contrast they can see.
