8. Review the Data Protection statement and provide a signature, along with the name in block letters of the signatory, noting the date of signing.
9. If applicable, indicate exemption from food charges by quoting the National Insurance Number and providing a signature.
10. Answer the health questions by ticking relevant boxes and attaching a Medical Declaration Form if necessary. Provide additional details regarding medication, allergies, dietary needs, and special requirements.
11. Complete the declaration section, confirming awareness of the need to be physically fit for the activity and that all medical matters have been disclosed. Provide the name and contact details of the participant's doctor.
12. Finalize the form by signing and dating it. Ensure all sections are filled out completely.
