- Complete sections 6 and 11 with the point of origin or Foreign Trade Zone number and the port of export, respectively.
- In section 13, indicate whether the goods are being shipped in a container for vessel transport.
- Provide the relevant details in sections 14 to 29, including the carrier identification code, shipment reference number, entry number, and any hazardous materials information.
- For the description of commodities, fill out section 20 using the appropriate Schedule B number and provide quantity, weight, and value in the following columns.
- Complete section 27 with any license number or license exception symbol if required, and provide the Export Control Classification Number (ECCN) in section 28 when applicable.
- Finally, ensure section 29 is signed by the duly authorized officer or employee confirming the truthfulness of the information provided.
