  9. Select the Gender by entering 'M' for Male, 'F' for Female, or 'T' for Transgender. This is a compulsory field.
  10. Proceed to outline the Date of Joining EPF, ensuring it is a valid date that does not precede the member's date of birth.
  11. Include the Date of Joining EPS and make sure it follows the same guidelines as the previous dates.
  12. If applicable, enter the Date of Exit from EPF in dd/mm/yyyy format, which cannot be earlier than the Date of Joining EPF.
  13. Fill in the Date of Exit from EPS, following similar formatting and rules as previous exit dates.
  14. State the Reason for Leaving by entering 'C' for Cessation, 'S' for Superannuation, 'R' for Retirement, 'D' for Death in Service, or 'P' for Permanent Disablement.
