13. Provide details of your domicile state and category by ticking the appropriate box.
14. Specify your exam center and fill in your previous educational qualifications, including the name of the examination, university/board, year, subjects, and marks obtained.
15. Finally, read the declaration statement carefully before signing. Ensure that all the information provided is accurate and true. Date the form and sign it in running handwriting.
16. After all fields are filled, save your changes, and you can choose to download, print, or share the completed form as needed.
