- List any Other Absences, such as Annual Leave or Long Service Leave, that coincide with the proposed program. Include the approved dates for these absences.
- Provide your Annual Leave and Long Service Leave balances as of January 1 or July 1, depending on your program's timing.
- Specify whether you will receive any additional Funds/Remuneration from other sources during your program. If so, attach the necessary details and signatures.
- List the names of any postgraduate students you supervise across various categories such as Honours and PhD.
- Detail your Current Research Topics.
- Outline your Proposed Itinerary, including dates, institutions, and contact persons. Provide evidence of arrangements for accommodation where necessary.
- In the Details of Proposed Program section, provide a General Description, Aims and Objectives, Proposed Outcomes, and the Benefits to yourself, your department, and the university.
- List your Publications from the previous six years, adhering to the required formatting guidelines.
- Sign and print your name in the designated area, along with the date.
- Complete Part B as your Head of Department, ensuring they provide necessary comments and keep you informed of arrangements made for your absence.
- The application needs to be signed by the Head of Department and forwarded alongside your Section A to the Dean of Faculty.
- After the review, collect comments from the Dean regarding program approval and provide your signature.
