11. If applicable, include any additional information that may assist with your application and provide relevant documents.
12. Confirm if your organization is registered for GST and supply your GST number.
13. Attach a budget projection for the next 12 months, as well as a statement of income and expenditure for the previous financial year.
14. Review the privacy notification section and indicate if there is any information you would like withheld.
15. Complete the declaration, ensuring that all key information has been disclosed and the information is accurate. Sign and date the application.
