8. In Section F, detail your estimated budget and all necessary expenditures, ensuring to follow the attached guidelines.
9. Complete the declaration in Section G. Confirm that the proposal is prepared under your supervisor’s guidance and is accurate. Sign and date the declaration.
10. Attach all required documents as listed at the end of the form. Ensure you have included two copies of the form, your supervisor's CV, flow chart, Gantt chart, milestones, and any evaluation forms.
11. Once all sections are completed and documents are attached, save any changes made, download your form, or print it for submission. Make sure to submit two copies to the appropriate office.
