Follow the steps to accurately complete your timesheet

1. Click ‘Get Form’ button to obtain the form and open it in the editor.
2. Begin by entering the month and year at the top of the timesheet. Ensure that these are correct, as they will identify the period for which you are reporting your hours.
3. In the 'Client's Name' field, enter the name of the client you are providing care for. This is important for tracking services accurately.
4. Fill in the caregiver's name in the 'Caregiver's Name' field. This identifies you as the person providing care and completing the timesheet.
5. Use the provided codes to indicate the status of the Plan of Care Tasks not performed, if applicable. Refer to the code list for accurate representation.
6. In the 'Services Schedule' section, input the date and corresponding 'Time In' and 'Time Out' for each day you provided services. Make sure to fill out all relevant time slots.
7. Calculate the total hours worked for each date and record this in the 'Hours' section. This is necessary for accurate billing of services.
8. Check the appropriate boxes for Respite, DHS, or Private Duty if any of these services were utilized.
9. At the bottom of the sheet, the client must sign the 'Client's signature' line, confirming that the tasks specified were completed.
10. You will also need to sign in the 'Caregiver's Signature' line, confirming that you worked without incident. If there was an injury, check the appropriate box and notify your supervisor.
11. Finally, input the current date and ensure both signatures are present before submitting the timesheet.
