How to fill out the APPENDIX 1: PIPP PAIN ASSESSMENT TOOL online

Filling out the APPENDIX 1: PIPP PAIN ASSESSMENT TOOL is a crucial step in accurately assessing pain in premature infants. This guide provides clear, step-by-step instructions to help users complete the form effectively and efficiently.

Follow the steps to complete the pain assessment tool.

  1. Click the ‘Get Form’ button to obtain the form and open it in the designated editing interface.
  2. Familiarize yourself with each indicator outlined in the tool, ensuring you understand how each should be scored.
  3. Before beginning the assessment, score the gestational age of the infant. This score is critical, as it adjusts the pain score based on the infant’s maturity.
  4. Score the behavioral state by observing the infant's actions for a duration of 30 seconds to gather accurate observations.
  5. Record baseline heart rate and oxygen saturation at the start. Monitor these over the assessment period.
  6. Observe the infant for another 30 seconds, noting physiological and facial changes. Ensure to record the observations immediately after this period.
  7. Calculate the total score by summing the points assigned to gestational age, behavioral state, heart rate, and facial actions.
  8. Interpret the total score: scores from 0-6 suggest minimal or no pain, scores from 7-12 indicate slight to moderate pain, and scores over 12 may suggest severe pain.
  9. Take appropriate actions based on the pain score, including potential interventions. Reassess the infant accordingly.
  10. Once completed, ensure to save changes, download, print, or share the form as required.

Score the neonate for each of the physiological and behavioural parameters, and for the nurse's perception of pain. Each item is scored from 0 to 2, and added to generate a total score out of 20 (the higher the score, the higher the level of pain).

The Neonatal Infant Pain Scale (NIPS) is a tool developed in the early 1990s [16] aimed to assess six behavioural reactions to painful procedures in preterm and full-term newborns. The scale was demonstrated to have high interrater reliability and internal consistency.

The premature infant pain profile (PIPP) is a validated pain scoring system for preterm neonates [2, 17]. For infants, non-verbal young children, and in patients with cognitive impairment, the face, legs, activity, crying, and consolability (FLACC) scale or the revised FLACC scale can be used [23–30].

Numeric Rating Scale (NRS) 0-10 This scale is normally used for children over eight years old. We ask your child to rate their pain from no pain to the worst possible pain. Sometimes, many factors can go into choosing that number.

The FLACC scale or Face, Legs, Activity, Cry, Consolability scale is a measurement used to assess pain for children between the ages of 2 months and 7 years or individuals that are unable to communicate their pain. The scale is scored in a range of 0–10 with 0 representing no pain.

Pain is most frequent and intense in infants admitted to the neonatal intensive care unit (NICU). Infants admitted to the NICU experience acute pain from skin-breaking procedures, short-term pain following surgery, and prolonged (chronic) pain from diseases like necrotizing enterocolitis or epidermolysis bullosa.
