9. Review and sign the certification in Part G to authorize the release of your information, ensuring all details filled are complete and truthful. Remember that if you are under 18 years old, your parent or legal guardian must also sign.
10. After completing the form, you can save your changes, download a copy for your records, print the form, or share it for submission to the SHEFA office.
