Follow the steps to accurately fill out your report

1. Press the ‘Get Form’ button to access the MS Constable Annual Financial Report and open it in your preferred editor.
2. Enter your name, mailing address, daytime telephone number, and employment status in the respective fields. Make sure to select whether this is an original or revised report.

10. In Part II – Disbursements, list all necessary expenses related to your office, including bank service charges, car and truck expenses, and office expenses.
11. For car and truck expenses, specify either the total actual expenses or the standard mileage calculation based on the number of miles driven.
12. Add up all disbursements from lines 8 through 23 to determine the total disbursements for the Constable and enter this figure.
13. In Part III, use the amounts recorded in lines 6 and 24 to determine your net fee income for P.E.R.S. by performing the necessary subtraction.
14. Proceed to Part IV – Retirement Contributions Calculation and fill out the employer and employee contributions based on the calculations provided in previous steps.
15. Continue with Part V and Part VI, ensuring you detail both the county retirement contributions and the Constable retirement contributions accurately.
16. Once you have filled in all sections, review your report for accuracy. You can then save your changes, download a copy, print, or share the completed form.
