5. The invigilator should also include their phone numbers (home and work), fax numbers (if applicable), business name, website, cell phone number, and email address.
6. Ensure that the invigilator is not a close relative or friend of the student and is qualified as per the specified professional categories.
7. Review all the information for accuracy and completeness. Once verified, save any changes made to the form.
8. After completing the form, download or print it if necessary. Finally, submit the completed form by faxing it to 604-936-6594 or emailing it to COLRegistrar@sd43.bc.ca.
