- In section 5, provide specific information to help locate the desired CCTV footage. You will need to specify the location of the CCTV camera, the date and time of the recorded image, and a brief description of the person's physical appearance and clothing.
- You should also provide a photograph of the person you are inquiring about. Acceptable formats include a passport-sized photo with their signature on the back or a copy of their valid passport or driving license.
- In section 6a, declare that the information provided is true. You must print your name, sign, and include the date. If you are under 16, section 6b must be filled out by a parent or person with parental responsibility.
- Once completed, send the form to the Data Protection Officer at NS&I, ensuring all necessary information is included. You can expect a reply within 40 days of receipt.
