As a final reminder, self-identifying as a person with a disability is certainly a personal choice, however, you do have the power to create opportunities for all of those with disabilities.

Identification: Employers can't make inquiries that directly or indirectly identify employees'and applicants' sex, gender, gender identity, or gender expression, unless employers establish a permissible defense; however, employers can ask applicants to provide this information voluntarily for recordkeeping purposes.

You can fill them in if you want, you don't have to if you don't want to. That's why they are voluntary. They are supposed to be either anonymous or confidential and not used to affect your file and/or hiring in any way.

Completing this form is voluntary, but we hope that you will choose to fill it out. If you are applying for a job, any answer you give will be kept private and will not be used against you in any way.

Identifying yourself as an individual with a disability is voluntary, and we hope that you will choose to do so. Your answer will be maintained confidentially and not be seen by selecting officials or anyone else involved in making personnel decisions.

Do I Have to Disclose My Disability? You do not have to disclose your disability during the application process if you don't feel comfortable doing so (if there is a question about it, it should clearly state that any information you provide is voluntary).

When asking employees to self-identify, employers should proactively communicate the purpose for the request and emphasize the confidentiality of the responses to help mitigate the discomfort or isolation that diverse employees may feel.

An invitation to self-identify is an optional question on a job application that allows the applicant to identify as a veteran or person with a disability. Some organizations also invite existing employees to self-identify using a standard form.
