Follow the steps to successfully complete your EBAHR request.

1. Click ‘Get Form’ button to obtain the form and open it in your preferred editor.
2. Provide your personal information in Section I. This includes your last name, first and middle name, alternate or previous names, and address. Ensure you type or print clearly.
3. Fill in your Federation Identification Number, USMLE ID Number, date of birth, phone number, email address, and national identification number if applicable. You can also provide your U.S. social security number and country of citizenship.
4. In Section II, calculate the required payment based on the number of EBAHRs you wish to request. Each request incurs a fee of $65 and allows for up to two EBAHR recipients. Indicate the total payment required clearly in the designated area.
5. In Section III, select which examination types (FLEX, USMLE, SPEX) you want included in your EBAHR. Be mindful that the Federation cannot provide scores for NBME or FMGEMS examinations.
6. Specify where you want your EBAHRs sent by providing the full name, address, and phone number of the recipients in part B. If you want any to be sent via express delivery, check the corresponding box.
7. Sign and date the authorization in Section III, part C, ensuring you understand and waive rights regarding the information provided.
8. Have the form notarized as required in Section IV. Ensure all necessary elements are included: notary's stamp, name, signature, commission expiration date, and date of notarization within the last six months. Photocopies of notarization will not be accepted.
9. Once completed, mail the notarized form along with the payment to the specified address using first class or express service. Make sure to retain a copy for your records.
