- Provide a summary of workplace-based assessments (WBAs). Remember that a minimum of three WBAs must be completed per quarter, and ensure copies are retained in your learning portfolio.
- Complete the clinical skills assessments section by filling in relevant topic areas, assessment dates, and the names of the WBA assessors.
- Fill out the management plan assessments in a similar manner, noting down relevant topic areas, overall ratings, assessment dates, and WBA assessors.
- Proceed to the case-based discussions and professional presentations sections, providing details of the relevant topic area, audience, and titles as required.
- In the comments section regarding WBAs, provide thoughtful reflections on your completed assessments.
- Discuss your progress in the clinical case study and document any observations you have made regarding strengths and weaknesses.
- Conclude by documenting areas of strength and weakness, specifically reflecting on your overall performance in practice.
- Your SoT/PDS supervisor will then assess your performance, providing comments on various competencies. Ensure that they fill out their section thoughtfully.
- Finally, summarize the discussion between you and your supervisor and outline your learning goals for the next ITA period.
- Complete the supervisor’s declaration section and ensure the supervisor's details are filled out accurately.
- Both you and your supervisor should sign and date the assessment before submission.
- Submit the completed form to the Faculty of Pain Medicine within 10 working days of the quarter.
