6. In Section 5, select your payout instructions. Indicate if you wish to receive cash (less taxes), obtain information about The Manulife Personal Plan, or have funds directly deposited into your bank account. Make sure to attach a ‘VOID’ cheque if you choose the direct deposit option.
7. In Section 6, review and confirm your understanding of the personal information collection. Sign and date the form to certify that the information provided is accurate and complete.
