4. Enter the 'Telephone Number' of the individual in the designated field within Section II.
5. Move on to Section III, where you must verify residency. Read the text carefully to understand the requirements for establishing your domicile in Texas. You must check the initial declaration confirming that you are a permanent resident of Texas and have lived there for at least 120 days before your production begins.
6. Choose one of the three document options that verifies your residency and fill in the required details for that document. This section requires specific numbers and dates related to the chosen evidence of residency, such as a Texas driver’s license, voter registration, or student ID.
7. The individual must then sign in the designated area, confirming the accuracy of the information provided. If the person is a minor, a parent or legal guardian should sign and note their relationship.
8. In Section IV, a representative from the production company must confirm the authenticity of the documents presented by the individual. This representative should sign and date the form.
9. After completing the form, review all entries for accuracy. Once confirmed, you can save your changes, download the form, print a copy, or share it as needed.
