Follow the steps to complete your application successfully

1. Press the ‘Get Form’ button to access the SC Mentor Certification Application and open it in your chosen editor.
2. In the mentor information section, fill in your name, South Carolina Bar number, firm or office name, complete address, phone number, and email. Also, include your date of admission to the S.C. Bar and any other jurisdictions where you may be admitted.
3. Indicate your S.C. Bar status by selecting the appropriate option: active, retired, or inactive. If you choose retired or inactive, provide the date of your status change.
4. In the mentor type section, check all applicable options for your mentoring capacity, including whether you will mentor a new lawyer joining your office or a separate new lawyer, or a group of new lawyers.
5. Designate the new lawyers you agree to mentor by providing their names and S.C. Bar numbers in the respective fields.
6. In the practice areas section, indicate your preferred areas for mentoring or check the option to mentor regardless of practice area.
7. Read the certification statement carefully. Sign and date the application, ensuring your SC Bar number is included, to certify your agreement to participate as a mentor.
8. Once the application is completed, save your changes, then download or print the form as needed. Finally, share or submit the completed application to mentors@scbar.org.
