14. Select the type of facility by checking the appropriate box indicating either existing permit coverage with no changes or new coverage due to significant changes.
15. Attach a topographic map of the facility's area, marking key features such as wells, rivers, and drinking water sources.
16. Attach a sketch of the facility to show treatment system locations for stormwater and any runoff characteristics of drainage areas.
17. Provide the average runoff in gallons per day for various areas on the facility.
18. Document the waste characteristics for each stormwater outlet by conducting pollutant analyses and provide results as necessary.
19. Confirm if a Storm Water Pollution Prevention Plan (SWPPP) or Groundwater Protection Plan (GPP) has been developed and retained on-site.
20. Submit a summary of the Discharge Monitoring Report data from any previous stormwater general permits if applicable.
21. Determine and indicate whether your facility has a pond that collects stormwater; provide details on the type and any oil/water separators present.
22. Finally, ensure to sign and date the application, certifying that you have reviewed and understood the information provided.
