1. Press the ‘Get Form’ button to access the form in an online format.
2. In box A, enter your social security number to identify your account.
3. In box B, fill in your employee name. Ensure that the information matches your official records.
4. Provide your agency name in box D and your unit information in box E to specify your work location.
5. If initiating a new ongoing payroll deduction, select section 1 and fill in boxes F and G, indicating your total monthly deduction, noting that the minimum amount is five dollars.
6. To designate nonprofits for your contribution, navigate to section 9. Here, input the nonprofit code number and name along with the designated amount for each organization, keeping the minimum limit in mind.
7. If you are changing a previous payroll deduction or organization, choose sections 2 or 3 accordingly, and specify new amounts in boxes H or J.
8. Indicate if you wish to contribute following retirement by selecting section 4 and filling in your desired deduction information.
9. For acknowledgment information to be released to nonprofit organizations, complete section 10 with the required details.
10. When all sections have been completed, validate your entries, then sign and date line 12 to authorize the payroll deduction.
11. Finally, review all entries for accuracy and save changes. You can then download, print, or share the completed form as needed.
