3. Provide your MJC student email and mailing address, including street name, city, state, and zip code.
4. In the 'School Information' section, fill out the school name and school phone number. Include the school address and your current grade level. Also, state your expected date of high school graduation.
5. Indicate which semesters you are applying for in the academic year, selecting from fall, spring, and summer options. Specify if the student is home-schooled.
6. List the courses you are interested in by providing the course name, number, and the corresponding units for each course.
7. The 'Certification by Principal or Designee' section must be signed by your principal or designee, confirming that you are academically prepared for the courses. Make sure they print their name and title as well.
8. In the 'Certification by Student and Parent/Guardian' section, both the student and a parent or legal guardian must sign and date the form. Ensure that the printed name and phone number of the parent/guardian are included.
9. Review all filled sections for accuracy. Once completed, you can save your changes, download the form, print it, or share it as needed.
10. Submit the completed form to MJCESeForms@mjc.edu.
