- In Part 3, Declaration of Authorized Officer, the officer representing your organization must complete their details and sign the declaration.
- Finally, in Part 4, the representative must sign their declaration affirming the information provided is true and complete.
- In Part 5, complete the Proof of Service section to confirm that you have mailed or delivered copies of the form to the appropriate USCIS District Director(s).
